Crane Wire Rope Inspection Requirements: What OSHA 1926.1413 Says Your Records Must Prove
Every crane company has a wire rope story. A rope that looked fine on Friday and had three broken wires in one strand on Monday. A drum layer nobody had eyes on in eight months. An inspector who asked for the monthly record and got handed a clipboard with four months missing out of the middle.
Crane wire rope inspection requirements are one of the most specific recordkeeping obligations in OSHA Subpart CC, and they are also one of the most commonly blown. Not because crane companies do not inspect. Most do. They blow it because the inspection happens and the record does not, or the record happens and nobody can find it nine months later when it matters.
This post covers what 29 CFR 1926.1413 actually requires, how long you have to keep each record, what the deficiency categories force you to do when something is found, and how to build a system that produces the paperwork without adding a second job to your competent person.
What OSHA 1926.1413 Actually Requires
The standard splits wire rope inspection into three separate obligations. They are not interchangeable. Doing the monthly one well does not cover you for the annual, and doing the annual does not cover a shift.
The shift inspection
Prior to each shift the equipment is used, a competent person begins a visual inspection, and it has to be completed before or during that shift. The inspection covers running and standing wire ropes that are likely to be in use during the shift, looking for apparent deficiencies. That is the wording in 1926.1413(a).
Note what the standard does not say. It does not attach a documentation requirement to the shift wire rope inspection the way it does to the monthly and annual. That does not mean you should skip the record. It means OSHA is not going to cite you for the missing paper, it is going to cite you for the condition of the rope, and you will have nothing to show that anyone ever looked at it.
The monthly inspection
Each month, an inspection is conducted in accordance with the shift inspection, and it has to include any deficiencies that the qualified person who ran the annual inspection determined must be monitored. That last clause is the one companies miss. The annual inspection can flag a condition as watch this, and once it does, that condition becomes a required item on every monthly inspection until it is resolved.
The monthly wire rope inspection must be documented under 1926.1412(e)(3).
The annual inspection
At least every 12 months, wire ropes in use have to be inspected by a qualified person. The standard calls for an inspection that is complete and thorough, covering the surface of the entire length of the wire rope, with particular attention to critical review items, including the sections that are normally hidden during the shift and monthly inspections.
Hidden sections is the operative phrase. The rope buried on the drum. The rope in the boom on a machine that never gets fully extended. If your annual inspection did not require anyone to spool off or extend, it was not an annual inspection. It was a longer monthly.
The annual inspection must be documented under 1926.1412(f)(7).
Crane Wire Rope Inspection Requirements for Recordkeeping
Here is where crane wire rope inspection requirements get concrete, and where most companies come up short.
Monthly inspection documentation has to be retained for a minimum of three months. The record has to show the items checked and the results of the inspection, plus the name and signature of the person who conducted it, plus the date.
Annual inspection documentation has to be retained for a minimum of 12 months, with the same content requirements: items checked, results, name and signature of the inspector, date.
Three months is a short retention window, and that is exactly why companies get burned by it. A three month retention floor sounds easy until you realize it means that at any given moment you must be able to produce the last three monthly records for every crane in the fleet, on demand, in the field, on the day someone asks. Not from the office filing cabinet a two hour drive away. Now.
The annual is worse in a different way. Twelve months of retention on a record that only gets created once a year means you have exactly one document, and if it is in a binder in a truck that got traded in, it does not exist.
The Deficiency Categories and What They Force You To Do
When an inspection turns something up, the standard sorts apparent deficiencies into categories and each category triggers a different response. This is the part your competent person needs cold, because the wrong call here is the difference between a corrected rope and a fatality.
Category I includes significant distortion of the wire rope structure, which covers kinking, crushing, unstranding, birdcaging, and signs of core failure, along with significant corrosion, electric arc damage, and improperly applied end connections. A Category I deficiency has to be corrected before that rope goes back to work.
Category II is where the broken wire counts live. Six randomly distributed broken wires in one rope lay, or three broken wires in one strand in one rope lay, is a Category II deficiency in running wire rope. A rope lay is the length along the rope in which one strand makes a complete revolution around the rope. If a Category II deficiency is identified, operations involving that wire rope are prohibited until the employer complies with the wire rope manufacturer established criterion for removal from service, or a different criterion the manufacturer has approved in writing for that specific rope. If the deficiency is localized, the problem can be corrected by severing the rope in two, and the undamaged portion may continue in service.
Category III deficiencies prohibit operations involving that wire rope until corrective action is taken.
Two things are flatly prohibited as corrective action. Joining lengths of wire rope by splicing is not allowed. And a wire rope that contacted an energized power line cannot be repaired. That rope is done. Severing it and keeping the good half is not an option when power line contact is the cause.
Where Paper Wire Rope Records Fail an Audit
Four failure modes come up over and over.
The record exists but nobody can produce it. The monthly inspection got done, the form got filled out, and the form is in the cab of a crane that is currently on a job three counties over. Retention means retrievable, not stored somewhere.
The signature is missing. The standard requires the name and signature of the person who conducted the inspection. A checklist with boxes ticked and no name is not a compliant record. An initial scrawled in a box is a fight you do not want to have with a compliance officer.
The monitored deficiency drops out. The annual flagged a condition to watch. That note lived on the annual form. The monthly form is a different piece of paper and it does not carry the note forward, so by month three nobody remembers to look.
The three month window closes quietly. Nobody is tracking whether a monthly actually happened on a machine that sat in the yard. A crane that was not in service that month does not need the inspection, but you need to be able to show it was out of service, and most companies cannot.
What a Wire Rope Inspection Record Should Contain
Build the form once and use it on every machine:
- Crane unit number, make, model, and serial
- Which rope: hoist, auxiliary, boom hoist, pendant, standing
- Rope specification and install date, so wear can be read against age and service
- Inspection type: shift, monthly, or annual
- Date and time
- Name and signature of the competent person or qualified person
- Every item checked and the result of each, not a single overall pass
- Any deficiency found, the category assigned, and the action taken
- Any condition the annual inspection flagged for monthly monitoring, carried forward automatically
- Photos of anything questionable, timestamped
That last item is not a regulatory requirement. It is what wins arguments. A photo of a rope section with a date stamp on it settles a dispute about condition that a checkbox never will.
What This Costs When It Goes Wrong
OSHA civil penalties did not receive an inflation adjustment for 2026, so the 2025 amounts carry forward. A serious violation carries a maximum of $16,550. Willful and repeated violations carry a maximum of $165,514 each. Those figures are published in the agency 2026 annual adjustment memo.
Per violation. A missing monthly record on four cranes is not one citation. The bigger exposure is not the check you write. It is what a willful classification does to your insurance renewal and your prequalification status with every GC who runs a safety screen before awarding work.
How to Build the System This Week
You do not need a six month project. You need four things.
First, a rope register. One row per rope per machine, with spec, install date, and service history. Most companies track cranes and have never tracked ropes as separate assets, which is why rope age is invisible to them.
Second, a recurring monthly task per crane that fires whether or not anyone remembers, and that a competent person completes on a phone at the machine, not on a form back at the office.
Third, monitored deficiencies that carry forward. When the annual flags a condition, that condition should appear on next month inspection automatically and keep appearing until someone closes it out with a documented corrective action.
Fourth, retention that outlives the truck. Three months and twelve months are floors, not targets. Keep everything. Storage is free and reconstruction is impossible.

Frequently Asked Questions
Does the shift wire rope inspection have to be documented?
The standard attaches documentation requirements to the monthly and annual wire rope inspections, not to the shift inspection. That said, an undocumented shift inspection gives you nothing to point at if a rope fails mid shift. Most crane companies that have been through an incident investigation document all three after the fact.
How long do I have to keep crane wire rope inspection records?
Monthly inspection documentation has to be retained for a minimum of three months. Annual inspection documentation has to be retained for a minimum of 12 months. Those are floors. There is no downside to keeping them permanently, and there is real downside to destroying them on schedule.
Who can perform each wire rope inspection?

Does a crane sitting in the yard still need a monthly wire rope inspection?
The monthly requirement attaches to equipment in service. A machine that was genuinely out of service for the period does not need the inspection, but you need documentation showing it was out of service. Absent that, a gap in the monthly record reads as a missed inspection.
